I think it depends on account registration date. My account @Cryptsy was created in 2013. And i got official notice from Receiver. And read other early registered users got it too.
While other users registered in 2015 and second half of 2014 did not get anything.

So probable DB backup was created somewhere in beginning of 2014. And notice was sent only to users registered before that date.

Yes, it worth to try.
I submitted my form yesterday. I do not have screenshot or video like some of others(only excel copy of balances), but i managed to dug up some of my old Cryptsy deposit addresses. And whole history of deposits is securely stored in according blockchain where it can be checked and confirmed independently.
It is a old addresses (created in 2013 and beginning of 2014), so it must be included in that old DB backup recovered by Cryptsy Receiver.
