Most important thing is not to send ETH/BTC from exchange,otherwise you might not get tokens.
Can you explain what you mean by this?
Exchanges operate a bit different so the wallet that you have on exchange is not suitable to be used as a wallet to receive tokens purchased from ICO projects.

I think it is safest to order from official websites of the wallet manufacturer like Trezor or Ledger.
