You didn‘t list PCS Infinity prepaid Mastercard:

This one has a free unique Iban account and can be applyed fully online. The account can receive and send (not 100% sure about sending) transfers from any EU bank account. I asked their support if crypto trades are ok and did not get any negative answer.
Should be mentioned here i think. But the card is 15€ per year, so not free!

Bitsa has a card program for teenagers between 14 to 17, even if you're not using cryptocurrencies you can top up your account balance with a bank transfer or with a debit card. There are virtual and physical cards but the thing is your parents may need to have their own account first.

You download the app, then send an invitation request to your parents, and they accept it, I believe that's how it works.

same thing happen to my friend, at least she hasn't got more than a few euros on the account, but a Premium account to be blocked and she was asked the same docs
this is connected to the anti money laundering policy... I am very careful with my revlolut account after that scenario, it's not a good thing for us as users

Using Revolut from fall 2018. From October 2019 I have a Metal plan (Premium user). I used my Metal card for several payments in the the store and small cash withdrawals (less than 300 eur), and my Revolut account for several transfers within EU (under 200 eur). I also used Revolut trading platform.
One day I've received a notification in Revolut app saying that my account is temporally limited for some security review and I need to provide some additional information to Revolut. Access to account was blocked. The only option I had was to contact Revolut support team via their in-app chat. After four hours of waiting one of the team member contacted me asking to verify my identity. Then they asked me questions about source of funds and the nature of the recent transfer. I've provided all the necessary information including pdf-documents required. The communication process was very slow and lasted for the whole day long. I was told that Revolut needs to check my account and when this check is completed they will contact me.
No terms, no specification of the reason for this blocking, no access to funds and to trading platform! They have just blocked my account with the balance of about 10 000 USD. It happened at the beginning of February.
I wrote them again several times via their support chat several times but received no answer. The chat does not connect me to any live agent. No reaction, no response, complete ignoring.
I have found them on Twitter. After sending a message asking about my issue I received a templated reply saying practically nothing new: account is currently under review and they can’t give me any further information. After that they just blocked me on Twitter as well so I can’t write them anymore.
After two months of waiting I have sent them an official email complaint and in 3 days received the same standard reply with no specific information about time frame and reasons.
It seems like they just don’t care about their clients, no matter if you are premium or standard user. No phone numbers to call. No respect.
After 10 weeks of silence I have sent another official email complaint to UK Financial Ombudsman Service and is now waiting for their reply.
Keep this information in mind and be careful with Revolut. They can do whatever they want whenever they want with your money, and you might have many problems getting it back. If you are a Revolut client try to find an alternative before they block you with no prior notification.

N26 and Fidor, other than that I wouldn't try any other. There are Wirex and Bitwala, it's not banks but you still get a dedicated IBAN to send and receive SEPA transfer.
If you read the topic you may find other suggestions and recommendations since your question has been asked several times in this topic

Does anyone know some KYC free debit card service?

Either Bitnovo or Bitsa card, I don't remember which one, has a card with 3 levels and the first level doesn't really require an ID verification (KYC). The user only needs to provide a mobile phone number, to receive an activation code I suppose.
The drawback is a low limit to reload, a maximum of €250.

Other than that, you can always exchange your bitcoins to buy a virtual debit card (visa or mastercard) via coinsbee.com and other alternatives.

It says on the website that they support USD/EUR only but IMO, If you're thinking of using their services though, I suggest you give it a second thought because except for a few articles[1][2][3] that were all published on the same date, they have no online presence which is very strange for a service that existed for over a year.

If the debit card is all you're interested in, there are much better and more reliable services out there.

They have a very detailed video KYC, I’ve done one before, for the Monese card, but this one was really detailed and long
