I agree with you that only an addict can help himself out of the addiction and that he needs external help to achieve that this is why we most times have to be guided in our approach when we come across gambling addicts because I have seen a situation where someone was publicly embarrassing his fellow simply because he can come out of his gambling addictions.
But hard it has been he is given the necessary help from family and friends, he will be better off and he should have been able to come out of his addictions.
But indeed, in some cases, like the family, they stay away from him (a gambling addict) even though he needs help to recover from the addiction which makes it very difficult to stop gambling, sometimes the addict also doesn't want his family to know he is a gambling addict which makes it very difficult to get outside encouragement.

I am not sure what "end-stage" gambling addiction is. I supposed you mean someone who has completely lost control of self and yet does not seem to even be aware of the problem, and that is the main issue when comes whether someone can recover or not from gambling addiction (or any kind of addiction), the person is supposed to know or realize at some point that they have a serious problem.
Still, I think that does not matter how bad an addiction is, there is a good chance to come back from it with the proper support. If there is people who could recover from addiction to opioids, then gambling addiction can be approached.
I agree with you, in other words if a person really wants to get rid of his addiction then there will be an opportunity for him, and he will definitely get healing even though it takes quite a long time because it is difficult to adapt to addiction which has become a character in humans.
