yes that's right, you should offer a certain amount of money to a woman's parents, but it's not mandatory. that must be done by a man there must be a marriage contract requested by the prospective wife.
That's right, if the man wants to marry another woman, he must get the approval of his first wife. not allowed, if the man marries another woman, but does not get approval from the first wife.
In Islam you can marry up to 4 women, sin / not permitted if more than that. and a man must be fair & wise to his wives.

I am not sure is there approval needed from the first wife or not,but there are some stories where first wife asked husbands to marry another woman because she ca't be able to give birth or she fell into some disease which is not curable.
But if a guy got two wife then he should treat both girl in the same way,if he failed to do then he is doing sin.
Islam allowed multiple marriages but there is no need we have to marry multiple women if we satisfied with one.
The first wife is the one that suggests to his husband to marry another. So, this really happens but with a valid reason, I guess. I watched a video of 'Asian Boss' from Indonesia related to the topic and IIRC, most of the women that were interviewed doesn't want their husbands to have another wife although they were aware of the rule.
