Just like the other companies in generally, something failed because of human source. How they market it, beside that the product that they are sell can be accepted by people with any reason. Of course there are still more reasons.
There are many reasons why some of them fail but instead of giving up on them you should search more of them and participate in many ICO as you can. In that way, you have still another to earn if the other one failed. The worst case scenario is falling into a scam ICO.
ico fails due to lack of investors for them and also the confidence in ico is reduced for now. more and more years ico appear, it could be because it triggered the emergence of ico scam that makes investors no longer interested in ico, therefore we need to be careful in choosing ico.
Some ideas of icos are not very good. Many think that just because they use a blockchain, they will succeed.

I think mainly ico fails because market is red, because of it they cant work under smaller budget and going broke easily after some time.
Most of ICO failed its because they have no unique project or even no good credibility of the Team. I've notice that some of ICO whitepaper are just copy paste from other project. The product they are offered is not special.

Maybe because of the project idea can't fit to the people desires, and some ICO are not highly budgeted and not well planned, so there's less investors will be interested to the project. Promotion is also has a big part of the ICO, the project needs to be well known as a legit one. People tend to invest on the project with news and updates with great engagement.

From my own point of view, most of these icos that fail are scam who are just out there to make quick money from innocent investors who knew nothing about their plans from the outset. Besides even those that are not scam but still fail are those without having a project with good future prospects backing up the ico.

too many ICOs that come this year, it is different like past when ICO not really a trend. if too many project and limited investors, so they can't get a lot of funds that they need to develop their project 
there are so many ICO`s in market that don`t deliver anything, don`t solve any kind of a problem and sell just dreams. This kind of project should fail and it would be a miracle for them to survive in long term. So it is normal for ICOs to fail.
Usually, they didn't reach the soft cap I believe it's due to the concept of the product that they're offering not attract investors, a project must have an innovative purpose nowadays investors are smart in choosing whether the project can them benefit or not. The other reasons they don't have a solid team and enough budget to promote the project, having a good team is a key here and big promotion can make ICO achieve the minimum target of the soft cap .

if it is then the sale did not reach the soft cap then the ico cannot be continued because the funds needed to run the project did not reach the minimum required

I think the main reason some icos falling because people lack of doubt because some icos are scamms and the investor afraid to invest and also icos falling affected of the crisis of the cryptocurrency value until now are covering in the market place their value.

Failing was a normal to those starting and i think the some and normalreason why they fail was lack of products, lack of research, strategy, man power or lack of knowledge in the project, planing, brainstorming or idea so that they are failing because if you have this all a best strategy best marketing i don't think that they are failing.
The main reason for unsuccessful campaigns is the lack of demand for the product offered by this campaign. There are many other factors that influence the success of bounty campaigns, and promotion of PR is also very important! In the world of the Internet and media, it is necessary to explain correctly why users need new technologies and development will not stand still and success will come with it. There is only the technical side of this issue and in many ways it depends on the competence of the team!

ICOs failed! This was happened because of the poor composition or the project group developers (team behind the project ) was didn't do their obligations and responsibilities very well. Sometime miss communications with their investors in one of the reason why ICO's failed because they didn't collect enough funds to run the project or to launch their project. Because of the poor negotiations of the team on the people they missout more opportunities.

I think the cause of ico failed is the product that does not make sense of us, the mission vision is not good and the price is not logical. Many ico entrepreneurs have less innovative products, as their investors prefer products that are in accordance with current technology to participant in investment, this certainly makes the main factor of investors in choosing ico. If ico does not meet the current technology, it will be very difficult to make the ico successful, and the team in the ico project development has good criteria that can make the ico project a success.
