as mentioned above, we cannot issue cards for Pakistan - it is a limitation imposed by the bank unfortunately..

unfortunately, Pakistan is not on the list of countries cards can be ordered and shipped to. This is due to banking compliance limitations

Can we deposit fiat money into the ATM machine and then have it converted to btc for some fee? or does it only work from btc-> fiat?
It is only BTC to fiat here.
What you are referring to exists, there are some bitcoin ATMs out there where you can buy BTC for cash, but they charge much higher fees (usually in the form of extremely high exchange rate)
