It's because people can't accept their mistakes. And they are in a defensive mode, very very hard to admit to themselves that they have make a mistakes and the possible consequences of it.
Or possible they don't want to hurt the feelings of the other party and just chooses to ignore everything as if nothing happens.

What are the most common reason why is it hard to say sorry to someone?
Because when you say sorry you accept a mistake and accepting things is something that some people find very hard, but it depends a lot on each person and on reason for repentance.
I think these reasons are the most common, but they are often difficult to accept:
1.- You are not used to saying words like that.
2.- Your pride maybe makes it hard for you to do.
3.- Do not accept that you had an error.

For me, it's damn hard to say sorry when I see that my mistake shows my incompetence or involves ego anyhow.
On the flipside, I can't help saying sorry to my close people in case I'm not sure who's falt it was. People matter more, in this case

Well, it think it's because everyone has a different mindset and everyone handles things in a different manner. So for example you might no feel to say sorry when you do a certain thing wrong because in your mind you don't see that thing as being wrong while other person thinks what you did was horrible and you should say sorry immediately. I don't find it hard to say sorry when I fell like doing it.
