You need to provide:

2. Token Symbol - ETB for example
3. Decimals (12 or 16 I do not remember).
Anyway when you are on your MEW you can click "Tokens (Ethplorer.io)" link and you can see all your tokens even not added as custom tokens to your MEW.

Its an ETH token so its unlikely that they would create a wallet when there are several good ETH wallets already.
