Well, nobody has brought up the reason why the Philippines is once again in the international news, so I thought I'd mention it, perhaps its easier for me to do so being the foreigner.

Understanding testing for COVID.

Warning, this WILL make your head hurt if you don’t have a science background. Heck it makes MY head hurt, but I cannot explain it any other way.

1. Mass testing is not testing everyone. It is “risk-based” testing. Basically, you test people in increasing circles of risk: test the PUI, then the close contacts, then the community. It is not a shotgun approach because no country can test every single citizen for COVID-19. So we need to figure out our priorities for testing, and WHAT TEST to use. You CAN’T test 100M people, but you can test the MOST AT RISK.

2. Understand the limitations of testing. No TEST is 100% accurate. There are trade-offs. The probability that a test is positive when the disease is REALLY present is called the SENSITIVITY. The probability that a negative test actually means the disease is REALLY NOT there is called the SPECIFICITY.

3. A good sensitivity means that a test is able to detect disease MOST of the time if it is PRESENT in a patient. Having a negative test when the disease is PRESENT is called a FALSE NEGATIVE. In other words, the test failed to detect a sick person.

4. A good specificity means that a test is NEGATIVE MOST of the time if there is NO DISEASE in a patient. Having a positive test when the disease is ABSENT is called a FALSE POSITIVE.

5. FALSE NEGATIVES are harmful because you say that someone is COVID-19-free when he actually has COVID-19 so that patient will be free to spread the disease.

6. FALSE POSITIVES are harmful because you will put a patient WITHOUT COVID-19 in the hospital, possibly with REAL COVID-19 patients such that the patient can get COVID, or be isolated needlessly.

7. So how good are the tests? There are two tests we can use for COVID-19 – RT-PCR and antibody tests.

8. RT-PCR is considered the best test for diagnosing ONGOING COVID-19 infection. PCR itself is very sensitive and specific, >90% for both. HOWEVER, the TYPE of specimen and the stage of disease (how many days with symptoms) can affect how often a test is positive. So for RT-PCR, using a nasopharyngeal swab in a patient WITH disease, the probability of getting a positive test is only 63%. So you will actually MISS 37% of cases. This is why we can do a REPEAT test after 48 hours in a patient who is getting sicker of what looks like COVID, but was NEGATIVE on the first test. The DANGER of RT-PCR is a FALSE NEGATIVE and you can end up clearing someone who actually has COVID-19. This can happen in UP TO 1/3 OF PATIENTS so its not a perfect test.

9. RT-PCR is also a highly technical process that not only involves having the right machine and kits, BUT also the proper SAFETY INFRASTRUCTURE like a BSL2 laboratory. Many labs and hospitals HAVE RT-PCR machines but they do not have the biosafety infrastructure.

10. Antibody tests include PRNT (Plaque reduction neutralization test, the gold standard), ELISA (enzyme linked immunosorbent assay) and lateral flow IgM/IgG. The first two are LABORATORY based assays and the last is a point of care rapid diagnostic test (POC-RDT).

11. As much as we would like to use rapid lateral flow assays (IgM/IgG) because of convenience, NONE of the lateral flow assays have used the industry standard PRNT assay as a gold standard. In other words, we have NO IDEA how good they are despite their claimed sensitivity and specificity. The biggest danger is that because it takes 5 to 10 days to make IgM antibody, the test has a high FALSE NEGATIVE rate in those who just started having symptoms. And so you will get a FALSE SENSE OF SECURITY and end up passing the virus to other people and your family members.

12. The OTHER problem with the lateral flow IgM/IgG is that there are other HUMAN CORONAVIRUSES that cause the common cold, and some antibodies against these viruses may CROSS-REACT with the test, giving you a FALSE POSITIVE, which is bad for the reasons stated.

The BOTTOM LINE is NONE OF THESE TESTS ARE PERFECT. FAR from it. Tests INFORM your response, but they still need to be INTERPRETED in the right context.

To the lay person, they think that a positive is a positive, and a negative is a negative. To us clinicians and scientists, they come with HUGE caveats in management. There are times we WILL NOT believe a test result because it is NOT CONSISTENT with the patient’s clinical picture. If we let ourselves be mislead by a test result without USING OUR BRAIN, people will DIE. And this also holds for doing public health strategies and mass testing.

I do hope this gives the public a glimpse into how complex these decisions are, and that no matter how much the public clamors for something, it isn’t always in their BEST INTEREST to push the issue over what is SCIENTIFICALLY SOUND. Thank you.

IATF Launches RapidPass System for use in checkpoint para malimitahan yung contact between mga frontliners/others allowed to travel natin at mga taong nag-mamando sa mga checkpoints. Ayon sa virtual presser ni Sec. Nograles kanina, tested na nila ito sa field with the help of PNP/AFP at i-roll out na mamayang hapon (4 PM according to the developers Devcon Philippines).

Under the system, frontliners in the checkpoints will use a QR code scanner to scan the unique QR codes that will be given to qualified or authorized individuals or vehicles. They can immediately pass the checkpoints after their codes are scanned.

The individuals exempted from the quarantine restrictions can request for a QR code through the RapidPass.ph. Once the request is approved, a unique and secure QR code and control number will be granted to each authorized person or vehicle.

The said QR codes can either be printed and placed on their vehicles or just show the codes on their smartphones to the frontliners manning the checkpoints.

Government eyes Mass testing by April 14 according to the chief implementer at the National Task Force on Covid-19, Carlito Galvez Jr. Ang focus muna ay lahat ng mga PUIs at PUMs. With more tests to be done, huwag na tayong magulat kung tataas pa lalo ang confirmed cases natin. On the bright side, mas mapapabilis na din ang pag-isolate at paggamot sa kanila.

I suggest na focus ka lang magbasa sa mga facts at huwag paapekto sa mga reklamo dahil Hindi mo din naman alam kung sinunod nila ng tama yung proseso.   

Ang mga tanong ko dito na hindi malinaw ay ang mga sumusunod na tingin ko dapat bigyan nila ng klaro.

  • Yung pag claim ng fund, masyado siyang vague, dahil ang nababasa ko ay thru bank account, ang pagpapadala sa mga approved nito, yung karamihan sa mga informal settlers, hindi gumagamit ng mga ganyan, so dapat ipadala nila mismo ito sa mga mabibigyan
  • Ang pinakaimportante, bakit may discrimination na dapat yung mahihirap lang ang makakatanggap or ipapriority? Masyadong unfair ito, dahil kahit yung mga may work or may kaya na tao apektado din at namomroblema din dahil sa issue. Yes priority ang mahihirap at need ng tulong pero, the fact na yung mga nagtatrabaho ay mga nagbabayad ng buwis, ay wala manlamg maramdaman sa ating gobyerno

Isa sa mga mungkahi niya dati ay yung pag-release ng maagang 13th month sa mga empleyado (pro-rated) at sa mga napagtanungan kong kakilala, binigay naman ng mga employers ito. Andyan din yung DOLE financial assistance.

May karapatan din naman magreklamo ang mga working class na nagbabayad ng kanila mga income taxes at hindi naman masisi.

200 Billion will be distributed to 18 million households for two months ~ 5,555 per household on average

Siguro ang next na tanong ay kung paano mag-claim? Kakailanganin niyo ng Social Amelioration Card (SAC) na manggagaling sa inyong LGU o baranggay. Naatasan din sila na magbigay ng master list ng qualified beneficiaries sa DILG. Kung sa tingin ninyo ay qualified kayo, magandang makipag-ugnayan kayo sa inyong baranggay at mabigyan ng SAC form.

Well, President Rodrigo Duterte promises that he will be announcing his address to the nation yesterday afternoon at exactly 4pm but he failed comply, he was late for about 7 hours and aired around 11 PM yesterday night. This might give a more negative image for the government simply because they are not following a simple notice.

Let me thank the LGUs who are doing their best to contain the spread of the virus in your localities. I see that you are employing innovative programs and initiatives to address the needs of your constituents.

Rest assured that the national government is behind you as we safeguard the welfare of our people in your communities.

I issued an order through Executive Secretary Medialdea clearly laying down the of each responsibilities department in implementing the Bayanihan to Heal as One Act. I have given them their marching orders, emphasizing the need for sufficient authority, so that they do not have to go back to me and ask for clearance for each and every action they have to take. All of the details of our actions will be part of my report, which will be submitted to Congress pursuant to Bayanihan Act.

There are now only two powers in the law that I reserved strictly for myself: One is the power to direct the operation of private establishments. The other is the power to require businesses to prioritize contracts for materials and services necessary for the crisis. I will exercise these powers only when absolutely necessary.

The government is now taking action and undertaking several key measures to overcome and recover from the effects of this pandemic. These measures have three main priorities : (1) Providing Emergency Assistance to Affected Sectors, (2) Facilities [and] Resources for Securing the Health Sector and (3) Performing Fiscal and Monetary Actions for the Economy.

FIRST: The government is now launching the largest and widest social protection program in our country's history pinakamalaki itong tulong na ibibigay ng gobyerno sa mamamayan sa history natin, eh ngayon lang tayo nagkaroon ng problemang ganito - to make up for the loss of economic opportunities due to the quarantine measures in place for COVID-19.

Beneficiary-households will receive emergency support for two months based on the regional minimum wage

However, I still count on everyone to also look out after their fellowmen --- their countrymen. To those who have more in life, I implore you to share in the cost of taking care of the less fortunate and keeping society our intact. They have also contributed to our prosperity in one way or [another]. So, let us support them in this crisis.

Let it be said that, in the hour of our country's greatest need, we all looked out for and helped each other.

I also appeal to the private sector to contribute, in any way you can, in dealing with this national emergency. We implore employers to do their part in protecting the welfare of the millions of Filipino households that belong to the formal sector and depend on their employers for continuous livelihood.

The government will provide a recovery and package—especially to micro, small, medium enterprises—to help deal with the economic effects of this pandemic. My economic team is already creating the guidelines for this as I speak.

To our farmers and our fisherfolk: We have not forgotten and is government you now employing quick response measures to help you during the crisis as well as ensure food productivity, availability, and sufficiency during the period of the pandemic.

The government is also ensuring the unhampered flow of cargo and their workers to maintain the continuous and steady supply of food, goods and other essential supplies and items. A price freeze on commodities is already in effect and measures to curtail hoarding and profiteering are also in place.

Food packs and essential non-food items are also being distributed [to] LGUs in Metro Manila and the rest of Luzon. Contaminated areas in the and Mindanao Visayas are receiving similar assistance. More will be given to augment the requirements of LGUs in the coming days as we repackage the items in our stockpiles.

SECOND: We are marshalling and deploying the resources of government to provide our frontline health workers with everything they require so they can save as many lives as possible.

We are now procuring medical supplies, devices and personal protective equipment.

About 1 million medical protective items are being acquired right now, with the government and the private sector working together to purchase or produce the necessary quantities.

We are also ramping up our capacity to test more broadly to take a fuller and more accurate picture of the spread of COVID-19 in the country. We will also build, operate and accredit more medical facilities and laboratories in the coming weeks.

The outpouring of donations and support from the businesses, organizations, foreign governments and private individuals over the past few weeks have also been overwhelming. I therefore like or would like to acknowledge the following for their help in this crisis:

Well, President Rodrigo Duterte promises that he will be announcing his address to the nation yesterday afternoon at exactly 4pm but he failed comply, he was late for about 7 hours and aired around 11 PM yesterday night. This might give a more negative image for the government simply because they are not following a simple notice.

Have you guys got your quarantine passes yet? In case you didn't know, they are being distributed to places under lockdown by barangay officials.
I have secured a home quarantine pass already, in our Barangay, only 1 pass per household also and I think this is a uniform implementation.
According to our Barangay Captain, we can use this pass to other municipalities or cities but I doubt they'll allow us to enter, the best thing our municipality would do now is just to secure the needs of the people so if this lock down will continue longer, we can have our needs within the municipality only.

And also, there was no social distancing anymore when buying goods in a grocery as people are rushing for their needs.

There's no confirm cases in our municipality yet but I am still afraid of the risk.
More like a same scenario here in our vicinity and people seems lax because no case has been announced yet. I guess hindi dapat ganito kasi yung symptoms ng kung sino mang nahawaan na only starts week/s after they've been exposed with positive carrying the said virus. Hoping lang talaga na the government will be stricter on implementing the ECQ and people realized that this isn't just a case to deny for.

Just because the cases are mostly on those regions said by nutildah that doesn't mean other places will just be lax on the implementation. I guess yung mga tao soon lang magiging serious sa sitwasyon if they found out there has been case/s na ibabalita which is not good to see because pwedeng kumalat na yan wala pa yung results.

Quarantine passes are currently suspended from where I'm currently from; and I really don't blame the local government. In a nearby city, quarantine pass queues are PACKED with people. It pretty much defeats the purpose of social distancing. Hopefully other cities are dealing with the queues a lot better. Manual delivery of quarantine passes to houses are probably out of the equation as it would require a good amount of manpower.

Have you guys got your quarantine passes yet? In case you didn't know, they are being distributed to places under lockdown by barangay officials. If they don't come to where you live, you can go to the local barangay office and get one there. You will need an ID and proof of residence.

If you live in a place that is not yet under lockdown, I would just assume its coming shortly (I guess unless you live way out in the country away from population centers).


They are giving out 1 per household, which anybody in the household can use to go buy food or medicine, but only 1 person can go out at a time (these rules I know apply to much of Luzon, Central Visayas and Northern Mindanao -- they were in Davao but they stopped). Right now it might not be such a big deal, but if lockdown lasts longer than mid-April, it might become a big problem later. I recommend keeping an eye out for that sort of thing in your local area.

Ours has been given one day before the enhance community quarantine take effect, our barangay officials give it house to house. I heard on other barangays in our city that some of the residents have to go to the barangay hall to get their HQP which for me is very risky of contamination of the disease and physical distancing rule is violated.

Is the HQP ^^ yours?

I've just read above that you are moving to Lapu-lapu City, meaning that HQP is yours. Welcome to LLC my friend. I can say that this place is not as risky as New York for now but still it's safe if we just stay at home.

Nasan na yung sinasabing 5k Financial Assistance, ng DOLE na sanay makakatulong sa mga ganitong tao. The effects of locked down clearly reduced the chances of everyone to raise money for daily living especially yung mga walang wala talaga. Nakakalungkot and nakakatakot, kasi pag tumagal pa ito ( possibleng extended naman talaga) eh hindi lang yan ang makikita natin sa news kundi puro holdapan at nakawan sa mga grocery or tindahan, at sa mga may kaya na tao.

Yeah may mga parang pagkukulang pero this is not really the right time to listen to criticisms from politicians who suddenly became experts in healthcare and medical stuffs. Wala pa ako nababasang reklamo o kritisismo mula kay Manny Pacquiao na siyang pinaka-visible sa mga Senador na tumutulong maliban kay Bong Go. The way I see it at this point, Gatchalian is not different to some rabid online social justice warriors out there (sorry sa supporters niya na makakabasa). Sana nga lang merong magaling na communications team ang DOH.

Nagbabasa ako ng posts ni Dr. Edsel Salvana, isa siya sa mga present sa mga meetings, at sinasabi nga niya na DOH and other agencies are working their butts of. He's been patiently explaining to the public yung tungkol sa proseso, which some politicians/journalists/online SJWs, don't understand. Hindi basta pwede madaliin yung mga request nila kagaya ng accreditation ng mga facilities at testing centers. Tama nga naman yung sinabi ng senador na time is of the essence pero may mga guidelines pa din na dapat sundin kasi nga buhay din ang pwedeng kabayaran kapag hindi nasunod ang safety protocols. Its already half a month of community quarantine and the numbers of cases is still growing here in the Philippines.

Resulta na din siguro ito ng expanded testing. Dumarami na kasi mga kagamitan na donated at purchased kaya mas marami na ang nate-testing ngayon (dati kasi targetted tesing lang nagagawa).



May nabasa akong post (a transcribed phone interview with the FDA head Usec Eric Domingo) na maaprubahan na ng FDA yung mga donated 10-minute rapid test kits as early as Monday. The approval was delayed kasi daw they're not as accurate as the test kits they are currently using. Mas malaki daw chance na magkaroon ng higher accuracy rate kapag gagamitin this third week. Anyway, mas maganda siguro kung mabasa niyo din yung mismong post I leave it to your judgement kung maniniwala kayo o hindi.

Suportahan naman sana natin lahat ng frontliners, maswerte tayong mga nasa bahay at hindi ganun kataas ang risk natin na magkacovid hindi tulad nila na talagang buwis buhay. Kaya sana huwag na makulit ang mga pinoy, kasi sa dami ng matitigas ang ulo eh pataas ng pataas pa din at dumadami ang nacoconfirm cases natin.

Im worried kasi ang sister ko is sa banking industry and I am concern sa kanyang health. Sana safe din ang mga kapamilya ninyo na frontliners mga kababayan.
Isama na dito ang mga couriers especially drivers na nagbubuwis buhay din in every pasada nila to deliver foods and other necessary things lalo na mga medical supplies.

Mabuti wala pang confirmed cases dito kung saan ako naka stay, though may mga PUI and PUM na, pero same thing to prevent the city nag ECQ na rin dito. Stay safe people, stay at home.

No, I'm not from here but have been living here for 2 years. Before that I was visiting a few times a year since 2016. I've lived in 4 provinces and 6 different islands. I get my visa renewed every couple of months, up until now -- can't get it renewed in time before it expires in early April. Apparently the Bureau of Immigration says foreigners have up to 30 days after the quarantine is lifted in order to get their next visa extension. So that's one thing positive out of this whole mess - I get a free month with no visa (at least, maybe two or more, but I hope not)!

There was a few stranded foreigners at the last hotel I was staying at. Some of them had flights cancelled with nowhere to go. One particular foreigner must have been totally without money, because the hotel gave him a job to clean for them! I didn't want to ask him his story... but I hope he can go back to his home country before too long.

Mainly I just commented in this thread so it shows up in my unread replies, so I can follow it as another news source. I lurk on this board sometimes and comment in the coins.ph thread when the company is giving me problems.  Cheesy
